Commonly used herbs include Scutellaria baicalensis, dandelion, honeysuckle, fructus forsythiae, Oldenlandia diffusa, Plantain seed and talcum, to eliminate internal dampness and heat, and improve the inflammatory environment of the epididymis.


- Yang deficiency and cold coagulation type (mainly characterized by cold pain, aggravated by cold, and fear of cold and cold limbs): The treatment focuses on warming the kidneys and assisting yang, and dispelling cold and promoting circulation. Commonly used herbs include cinnamon, fennel seed, Radix rehmanniae praeparata, dogwood, Eucommia, and are combined with blood-activating herbs such as cassia twig and safflower to dispel cold and promote blood circulation.


- Phlegm–blood stasis type (mainly characterized by epididymal nodules and relatively hard texture): The treatment focuses on promoting blood circulation and resolving stasis, and softening and dispersing nodules. Commonly used herbs include Salvia miltiorrhiza, peach kernel, Radix paeoniae rubra, rhizoma sparganic, curcuma zedoary, as well as seaweed, Ecklonia kurome, carapax trionycis, fritillary bulb, and selfheal, to eliminate nodules and unblock the meridians.


Examples of commonly used prescriptions: In clinical practice, classic prescriptions such as Juhe Wan, Xiao Yao Powder, Tian Tai Wu Ye Powder, Long Dan Xie Jia decoction, Nuangan decoction, and Cassia twig tuckahoe Capsule are often selected or modified for use. Or Experience-based formulas such as Yi Qi Huo Xue San Jie Tang (“Decoction for Tonifying Qi, Promoting Blood Circulation, and Resolving Nodules”) may also be used. Patent medicines like the Diuretic and Anti-inflammatory Pill and Ju He Wan are commonly prescribed, as they combine multiple effects—clearing heat, detoxifying, promoting blood flow, and softening masses—while being convenient to take.


II. Integrating External and Internal Therapies: Delivering Medicine Directly to the Affected Area

The external treatment method can directly apply the medication to the affected area, complementing the internal medication and enhancing the therapeutic effect.


External application of traditional Chinese medicine: Select traditional Chinese medicine ointments or powders with the functions of clearing heat and detoxifying, reducing swelling and relieving pain, such as Ruyi Golden Yellow Ointment and Golden Yellow Powder. Apply them externally to the affected scrotum. Modern research also shows that local external application can promote drug penetration and alleviate inflammation and pain.


- Traditional Chinese medicine steam bath and sitz bath: Choose different prescriptions to make decoctions for steam bath or sitz bath according to the syndrome type.

- For cold congealing type, you can use herbs like mugwort, cassia twig and safflower to warm the meridians and dispel cold.

III. Acupuncture and Massage to Unblock Meridians and Regulate Qi and Blood Flow 

Acupuncture and massage are unique non-drug therapies in traditional Chinese medicine. They achieve therapeutic effects by stimulating specific acupoints and meridians. 


- Acupuncture treatment: The doctor will select relevant acupoints such as Guanyuan, Qihai, Sanyingjiao, Taichong, and Shenyu based on the diagnosis, including those on the liver meridian, kidney meridian, and ren meridian. - Daily life and rest: Avoid prolonged sitting and long periods of cycling, and reduce pressure on the perineum. Wear loose and breathable cotton underwear to keep the genital area clean and dry.


- Dietary adjustment: Avoid spicy, greasy, and stimulating foods. For those with damp-heat syndrome, it is advisable to consume more light and diuretic foods (such as winter melon, green beans); for those with yang deficiency, it is advisable to warm and tonify, and avoid cold and raw foods.


- Moderate exercise: Such as walking, Tai Chi, etc., to enhance physical fitness and promote the circulation of qi and blood.


- Regular check-ups: Keep in communication with the doctor and adjust the treatment plan in a timely manner according to the changes in the condition.


In summary, the treatment of chronic epididymitis with traditional Chinese medicine is a comprehensive process.
